How to create Questionnaire

The Questionnaire Master is used to create and manage structured sets of questions by organizing them into questionnaire groups. Within each group, multiple questions can be added using various response types such as Text Area, Checkbox, Radio Button, Rating Drop Down, Star Rating, Smiley, Thumb Rating, and Heart Rating. This flexibility allows capturing different kinds of feedback and inputs effectively. Once the questionnaire groups are created, they can be mapped to specific system modules. Commonly, these groups are linked to modules like Confirmation and E-Separation to collect relevant employee responses during those processes. This ensures that the right set of questions is presented at the appropriate stage. Overall, it helps streamline data collection and improves process-specific feedback management.

We create Group by using Add button, Then Configure or add the set of questions from the Add question button.

How to create Questionnaire ?

Navigation – Click on HR Corner –> Click on Questionnaire Module –> Click on Add Button.

Fill Group Details and Create Questionnaire Group.

Questionnaire Group created.

Click on Add “Question Button”.

Click on “Add Question” Button.

Add Question and then by clicking on Add row button you can add Answers options.

After adding all the questions, Questionnairs looks like below Screen.
